Before your first session:

Before we begin our work together, I ask that you fill out the following forms. 

You will receive an email where you can fill out and sign the forms below.

 

During your first session:

During your first session, we will get to know each other better and I will review some of the important policies and procedures that cover topics like informed consent, payment, and cancellations/no-shows.

 

We will spend the majority of the first one to three sessions talking through the intake form, which provides a detailed overview of your wellness history. If we haven't already, we will set our schedule moving forward, based on mutual availability and assessed need. 

Before the second session, I will ask you to sign several forms, including the informed consent previously discussed policy forms.

 

Before your second session:

Before your first session, you will need to complete and sign the following forms. 

You will receive an email where you can fill out and sign them.

 

Make a Payment

Payments are due on the day of your scheduled session, before your session time.

 

You may make payments via credit card by setting up Auto-Pay through Alma. 

You may also make payments via Venmo to Grace-Huntley-1.
